School-Based Telehealth Consent Form
St. Elizabeth Physicians is offering School-Based Telehealth Services. These telehealth services do not take the place of your primary care provider. All students, faculty, and staff are welcome to participate in this program. You do not have to be a current patient of St Elizabeth Physicians and will be encouraged to have any needed follow up care with your primary care provider. What is virtual
school-based health?
Should your child get sick at school, virtual school-based health is a way to have your child seen by a St. Elizabeth Physicians provider through video conferencing. The provider is then able to diagnose your child and call in prescriptions to your pharmacy, if needed.
What happens if my child gets sick at school?
Your child will go to the school nurse’s office to be examined. If the nurse determines that a telehealth visit with a St. Elizabeth Physicians provider is appropriate, the parent or guardian will be contacted first.

St. Elizabeth Physicians patient?
No, all students are welcome to participate in the program. However, preregistration in the program by the parent is required. An after-visit summary will be sent to your child’s primary care provider.

What illness can be treated in a virtual visit?
Many conditions can be diagnosed and treated through telehealth, ranging from minor acute issues such as sore throat, ear pain or injury to reoccurring chronic issues like asthma. If the provider feels that your child needs additional testing or an in-person visit, they will help guide appropriately. If additional testing is warranted, school nurse may facilitate testing for certain conditions if available.
Why should I enroll my child in this program?
This program offers the convenience of having your child diagnosed by a healthcare expert and prescriptions sent to your pharmacy, all while remaining at school and in most cases, you remaining at work.
Will I be contacted before the visit?
You will be contacted by the school nurse with the appointment information as well as an invitation to participate via 3-way video.
What if I am unable to join this visit, how will I know what the provider recommended for my child?
If you are unable to participate in the visit, a St. Elizabeth Physicians team member will contact you to review recommendations.
Will I be charged for this service?
The virtual visit will be billed to your insurance on file, similar to a standard office visit. Please check with your insurance carrier for coverage of Telemedicine Video Visits. If your child does not have the insurance, we’ll send you an invoice for the cost of the services.
Does my child always have to see the virtual provider if they go to the nurse’s office?
No. While we do encourage enrollment, you do have the option to decline the video visit when you are contacted that day.
