Free Interpretation and Translation Services for Our Patients
St. Elizabeth Physicians will take reasonable steps to ensure that persons with Limited English Proficiency (LEP) have meaningful access and an equal opportunity to participate in our services, activities, programs and other benefits.
St. Elizabeth Physicians ensures meaningful communication with LEP patients/clients and their authorized representatives involving their medical conditions and treatment. We also provide for communication of information contained in vital documents, including but not limited to consent to treatment forms, financial and insurance benefit forms, etc.
All interpreters, translators and other aids needed shall be provided without cost to the person being served, and patients and their families will be informed of the availability of such assistance free of charge.

Nondiscrimination and Accessibility Requirements
St. Elizabeth Physicians does not discriminate on the basis of race, color, national origin (including limited English proficiency and primary language), sex , age, or disability.
St. Elizabeth Physicians provides reasonable modifications for individuals with disabilities, and appropriate auxiliary aids and services, including qualified interpreters for individuals with disabilities and information in alternate formats, such as braille or large print, free of charge and in a timely manner, when such modifications, aids, and services are necessary to ensure accessibility and an equal opportunity to participate to individuals with disabilities.
St. Elizabeth Physicians provides language assistance services, including electronic and written translated documents and oral interpretation, free of charge and in a timely manner, when such services are a reasonable step to provide meaningful access to an individual with limited English proficiency.
Anyone who requires an auxiliary aid or service for effective communication should contact the Language Services Administrator at (859) 655-9326. If modification of policies or procedures to participate in a program, service, or activity of St. Elizabeth is required, individuals should contact the 1557 Coordinator, at (859) 655-7105, as soon as possible but no later than seventy-two (72) hours before the scheduled event. The ADA does not require that St. Elizabeth take any action that would fundamentally alter the nature of its programs, services, or activities, or impose an undue financial or administrative burden.
Any person who believes someone has been subjected to discrimination on the basis of race, color, national origin, sex, age or disability may file a grievance under this procedure. It is against the law for St. Elizabeth Physicians to retaliate against anyone who opposes discrimination, files a grievance, or participates in the investigation of a grievance.
St. Elizabeth Physicians has adopted an internal grievance procedure providing for prompt and equitable resolution of complaints alleging any action prohibited by Section 1557 of the Affordable Care Act (42 U.S.C. § 18116) and its implementing regulations at 45 C.F.R. pt. 92, issued by the U.S. Department of Health and Human Services. Section 1557 prohibits discrimination on the basis of race, color, national origin, sex, age or disability in certain health programs and activities. Section 1557 and its implementing regulations may be examined in the office of Katie Koch who is designated to coordinate the efforts of St. Elizabeth Physicians to comply with Section 1557:

Grievances must be submitted to the Section 1557 Coordinator within (60 days) of the date the person filing the grievance becomes aware of the alleged discriminatory action.
A complaint must be in writing, containing the name and address of the person filing it. The complaint must state the problem or action alleged to be discriminatory and the remedy or relief sought.
The Section 1557 Coordinator (or her/his designee) shall conduct an investigation of the complaint. This investigation may be informal, but it will be thorough, affording all interested persons an opportunity to submit evidence relevant to the complaint. The Section 1557 Coordinator will maintain the files and records of St. Elizabeth Physicians relating to such grievances. To the extent possible, and in accordance with applicable law, the Section 1557 Coordinator will take appropriate steps to preserve the confidentiality of files and records relating to grievances and will share them only with those who have a need to know.
The Section 1557 Coordinator will issue a written decision on the grievance, based on a preponderance of the evidence, no later than 30 days after its filing, including a notice to the complainant of their right to pursue further administrative or legal remedies.
The person filing the grievance may appeal the decision of the Section 1557 Coordinator by writing to the Administrator within 15 days of receiving the Section 1557 Coordinator’s decision. The Administrator shall issue a written decision in response to the appeal no later than 30 days after its filing.
